HTML Email Forms

Im looking to know if anyone could point me in the right direction for a solution to this:

-Need to create an email form which can be completed by the user

-Form can then be previewed and printed out (maybe open another window displaying the information?

-Once user has verified information is correct they submit it and its mailed to us and they can also mail it to themselves for their own records

-And lastly we need their customer ID put into the email subject line


Basically its a returns request form for customers where they fill out the information and mail it to us so we can track customer returns and faults.

You're going to be able to do something fairly simply in PHP. If you don't know it, it doesn't take too long to learn, or I'm sure someone on here could knock something up fairly quickly.

Ive managed to get the PHP form working and emailing with the customer Id in the subject field.

The only stumbling block is getting a printable version of the form for the customer to print out.
