The first one came off due to me having to run a 10mm spacer behind the adapter, as the adapter was fouling the brake cradle, resulting in me not having enough thread on the bolt for it to be secure enough on the hub. Im not running the 10mm spacers anymore as I have diffrent hub adapters, so...
Do not have a clue! the adapter was still bolted onto the wheel, so the bolts inide the adapter have come away from the hub, can only quess I didn't do those up tight enough when I last had the wheel off and they've worked themself loose

The opposite one decided to come off, have no idea why it done it, in all the times ive had the wheels on, they havn't fallen off (with exception of the first time lol)
