Car lurching forward whilst breaking

Afternoon All,

Sometimes when slowing down using the brakes, in gear, the car will lurch forward whilst breaking.
The revs aren't low enough for the car to be stalling when doing so.

Have yet to actually check anything, thought it's worth an ask.

Change down a gear then or depress the clutch.
