If your car is on high miles or making a noise then it's best to change it or just change it for piece of mind.

If you are near (or over) 100K and it hasnt been done yet, do it the next time you change the belt.
Or do it if its noisey.

its done 52k? its going in for its first belts change soon but the dephaser will cost an extra 160 on top. just wondering if its worth getting it done this time or next time belts are due?
 
No one can really say get it done or don't. If it's noisy then definitely 160 well spent. If it's quiet it's kind of at your discretion. The next time you'll be having both belts according to schedule will be 124K or 2017... (5 years/72K whatever's first). If it goes before then obviously you'll need the whole lot doing earlier.

If your having your belts done change the dephaser, you will only pay again if it's noisy just after te cambelt change, this does happen.
