All you have to do is go to the o2 website and enter in your phone number and new sim card number, you get a text with a code which you enter into the site and the "sim swap" starts. Mine took a couple of minutes. Probably take you longer to cut the sim!

As long as you have your iTunes library backup it'll all work smoothly. The iPhone is linked to that iTunes library so by restoring the library you'll restore the phones syncing ability.

It's the monitor only accepting specific inputs via HDMI. VGA inputs on TV's/monitors tend to accept a much wider range of input resolutions, one of the VGA resolutions will be native. Connect the MBP via a VGA adaptor and it'll work as well as the windows laptop did.
