Failing emissions

What would the main cause/s be for emissions being to high. Got a mate of a mate whos car failed twice now and it failing by 0.12 if that means anything

Lamba Sensor i'd imagine. I think 0.12 is 4 times too high if my memory serves me correctly.

ensure the car is thoroughly warmed up before putting it in for an emissions test, the catalytic converter's effiency is very poor at low temps

make sure there aren't any leaks/blows in the exhaust

then after checking these i would start looking at spending money
