IT Service Contract (very small business)

Can anyone point me in the right direction (web link) where I can download free IT service support contacts?

I'm s**t at writing them, need to knock up a little one which has all the basic info on there like pricing per out, call out fee, T&C etc etc.

Any help is necessary
