Lumpy idle and engine rattle when pulling off

I'm thinking that's probably what it is, it rattles when I start the car too, but would that explain a lumpy idle?

Does the rattle diappear at certain revs?could be a dephaser with the rattle but wouldnt explain the lumpy idle
