Dont take it anywhere other than Tmobile, T-mobile are the only people who can unlock other companies will just scramble the software in the phone, potentially causing damage! The damage could be very little eg the occasional glitch or it could literally completely nacker your phone

no you just need the code from T-mobile, and maybe the number it originally came with. I work for O2 not t-mob so not 100% sure. sorry
