The only one I have come across was Zen, doesnt apply now they dont do an Unlimited service. Back when I had a 2MB connection at home it was on nearly 24/7 but you paid for the privilege.

Never had problems even with them behind a firewall and most ports locked down. I would imagine it just went through something like 80 to avoid any problems.
