Are both rear wheels locking?
I'm thinking you may have a seized shocker or a twisted axle that is unloading one wheel (like cocking a leg)
When heavy braking is causing a lock on that wheel
Just to add I would put the brake lines back in there original ports (diagonal split) as I don't fancy...
