Clutch help!!!

My dad borrowed my car today and when he came back he said my clutch is slipping??!? i dont really know much about cars. He said i might need a new one (how much will this cost?) or it might need tightening. I've looked through the service history and theres nothing mentioning the clutch so it might still be the original one.

My car is a 1994 M Reg Clio RT.

Any help people???
