Custom made means that the exhasut is welded and fitted to the car on site at manufacture.
This does involve having to leave your car at the garage for a few hours when this is done. But the wait is worth it.

You have to remove the dash then behind the display (clock and temp. gauge) you need to pull apart that connector and plug in the adapter as you described above.
This adaptor also requires its own power supply, just the supply for the radio - IMO
