to be honest i would not bother reporting it to insurance take it to some little bodyshop and they will fix it cheap for u . or if you cannot afford it put a claim in some times boot floors can be pulled back into shape fairly easily others it may need replacing .
try egr valve there prone to goin on these . iirc that gave similar symtoms . it puts exhaust gases into engine to help start when cold . so would explain why it starts ok when warm . the smoke will be unburnt fuel and then it all burns when car first starts = bit of smoke . Matt
