Any idea when it came off? i.e have you noticed a load of grease over the inside of the wheel at any point?
If I were you, get yourself a tub/tube of molly grease ASAP, get a jubilee clip, get some more grease inside that joint and re-clip the bloody thing!

Mr Duck, It had a new drivers side shaft about 9 months ago, which it turned out it didn't need, so I've got a spare drivers one lying around in the garage if I really need it.

Well yes.. Sort of... The box side of things has to engage/disengage from things, so when pulling away in first the inner joint would surely experience more forces/friction than the cv joint? Which pretty much puts up with the same thing all the time, unless wheel spinning etc...
So either...
No difference, ever, just first gear, pull away hard. (It doesn't even do it when rolling in first, so if you pull of gently, then floor it, it's fine.)
