Help with copy and pasting

I need to copy just one page from the below document but need it as shown on the page, but every time I try and do it it comes out without the boxs and just lists the info down the page.

I know u guy's are clever so can anyone do it for me cheers

Do have access to Adobe Acrobat? or just Adobe Reader?

If you have Acrobat, all you have to do is:

File>Print> Chooose Adobe PDF from the printer list > choose the page you want to "print" and click ok then it will ask you for a file name and save it
