How does buying a second hand iPhone work r.e. Sim cards?

Give PAC code to o2 to get my number on iPhone sim card
Start a sim-only contract with o2 on the iPhone's sim card

Any hidden charges, complications etc?

That doesn't really make sense.

You get your PAC code from whoever, then you get an o2 simcard and they'll transfer your number.

There's nothing else to worry about, unless it's an iPhone 4, in which case you need to make sure you get a micro-sim, but they don't cost anything.

If you are getting an iphone contract with them then there are no issued. but you can buy a simcard cutter to turn your sim into a micro sim from ebay, not expensive either.
